Immune status of representative infectious diseases among Japanese female university studentsMisaki Katsuyama; Yuji Koike; Toshie Hirohara; Kazuhiro Kogawa, Corresponding, Objective: To elucidate the immune status of representative infectious diseases among Japanese youth, we retrospectively investigated serum antibody levels in university students, partly comparing these to immunization records and infectious disease histories confirmed by the maternal and child health (MCH) handbooks. Materials and methods: In total, 168 Japanese female university students, aged 20-21 years, were included. Data were collected from examinations of antibody titers against measles, rubella, varicella-zoster (VZ), mumps, and hepatitis B (HB) and C (HC) viruses, and from QuantiFERON®-TB Gold tests, between 2011 and 2015. Records of immunization and infectious disease histories were available from MCH handbooks for students who agreed with the use of their data for this study (n=23). Results: All students had positive antibodies, detected by enzyme immunoassay (EIA), against measles, rubella, VZ, and mumps
however, seroprevalences within the range of seroprotective antibody levels were 38.1% (64/168), 67.9% (114/168), 95.9% (141/147), and 89.8% (132/147), respectively. The students had probably not been infected with HB, HC, or tuberculosis at the time of the examinations. Discussion: The study indicated that a two-dose vaccine for measles and rubella (MR) might not be sufficient to produce antibodies at seroprotective levels. Therefore, we propose that health care workers, including students, should receive an additional MR vaccine, even if they have received two doses of MR vaccine or if they have unknown histories of immunizations or infectious diseases. Participants were second- to fourth-year students (n=945) at Ibaraki University, Japan, who were randomly chosen for the survey to measure their attitudes toward lesbian, gay, bisexual, and transgender (LGBT) individuals. We divided the respondents into two groups: one group who had encounters with LGBT people, and one group with no such experience. Chi-square tests were used to compare responses between the groups. More than 60% of the students considered LGBT people to be acceptable
the group with prior encounters had greater levels of acceptance compared to the group without such encounters. This is the first study to clarify the acceptability of sexual minorities among university students in Japan. [Reviewed] Transitional changes in the prevalence of dental caries in children and preventive strategies: A review of nationwide annual surveys in JapanChizuru Mukouyama; Yuji Koike; Toshie Hirohara, Corresponding, Purpose: To elucidate the efficacy of strategies for preventing dental caries in Japanese children by examining trends over time in the prevalence of dental caries, the number of decayed, missing, or filled teeth (the DMFT index), and their association with factors affecting oral hygiene. Materials and Methods: Several national surveys performed between 1955 and 2015 were reviewed. The prevalence of dental caries in children aged 5-15 years, the DMFT index in children aged 9 or 14 years, individual consumption of added sugars and snacks, the number of dentists in Japan, and the percentage of children who received fluoride varnish were analysed. Additionally, the relative poverty rate among Japanese children was assessed. Results: The prevalence of caries and the DMFT index in Japanese children reflected the trend of individual addedsugar consumption from 1955 to 2015, and also seemed to decrease inversely with the number of dentists and the percentage of children who had received fluoride varnish since 1969. However, there seemed to be no relation to the relative poverty rate. Conclusion: The main preventive strategies for caries in Japan are to reduce sugar intake and educate school children and their caregivers about oral hygiene., Quintessence Publishing Co., Ltd

[Reviewed] Successful treatment of short stature with growth hormone replacement therapy in a patient with anorexia nervosaYuji Koike; Masaya Akibayashi; Yukako Yokouchi, Lead, A 19-year-old woman visited our outpatient clinic requesting treatment for short stature. She had been repeatedly hospitalized at a psychiatric unit and was subsequently diagnosed with anorexia nervosa (AN). She was 139.3 cm (-3.6 SD) tall and weighed 25.5 kg (23% lower than standard weight). She had primary amenorrhea and her bone age (BA) was 11.8 years. She had low insulin-like growth factor (IGF)-I (80 ng/mL) and a basal growth hormone (GH) level of 1.47 ng/mL. Treatment with recombinant GH was initiated. At 22 years of age, she was 152.2 cm (-1.1 SD) tall and weighed 39.7 kg. As she had shown a favorable response to GH treatment, therapy was discontinued. We suggest that it is worthwhile treating AN patients with GH replacement therapy for short stature, once low IGF-I levels without GH resistance, delayed puberty, delay in BA, and nutritional stabilization are taken into consideration., De Gruyter

Materials and methods: Participants were first-year students (n=1597) at Ibaraki University, Japan (Phase 1 study) or second- to fourth-year students (n=944) at the university who were randomly preferred in the survey (Phase 2 study). Surveys measured gender identity and sexual orientation, partly using the gender identity scale (GIS). Results: The prevalence of LGBT youth among university students were 2.7%, 0.5%, 5.3%, and 0.8% (Phase 1 study) or 1.4% (Phase 2 study), respectively. The GIS scores of the transgender group were significantly lower than those of the female, male, and LGB groups (p<
0.01). Discussion: This is the first to clarify the prevalence of LGBT among youth in Japan. It has been suggested that such individuals in Japan, as in other countries, are at risk for physical and mental health concerns, thereby necessitating social and medical intervention. Late, late-onset group B streptococcus cellulites with bacteremiaYokouchi Y; Katsumori H; Koike Y., Corresponding, Group B streptococcus (GBS) infection remains a leading cause of serious neonatal and early infantile infection. As the infection often presents with nonspecific symptoms, and is associated with underlying bacteremia, prompt investigation and treatment is required. We report a case of late, late-onset GBS infection with bacteremia in a 94-day-old boy experiencing cellulitis of the left hand. Although late-onset disease or late, late-onset disease has been reported to be common among infants with underlying conditions such as premature birth, immunocompromised status, trauma, or among those using medical devices, no such underlying medical condition predisposed this infant to invasive GBS infection. Recent reports including the present case underscore the risk of GBS infection among previously healthy infants beyond the neonatal period. Protective effects of influenza A (H1N1) pandemic 2009 vaccination against the onset of influenza-like illness and asthma exacerbation in Japanese childrenYokouchi Y; Katsumori H; Shirakawa S; Fujiwara M; Kashima K; Kozawa R; Koike Y., Last, Background: Vaccination against influenza A(H1N1)pdm09 in Japan started in October 2009. Children with asthma are considered as a high-risk group and are recommended to preferentially receive the vaccine. Objective: To identify the clinical effects of vaccination in Japanese children with and without asthma. Methods: We conducted a cross-sectional, questionnaire-based survey to compare vaccination rates, vaccine effectiveness against physician-diagnosed influenza A infection (PDIA), and consecutive asthma exacerbations between children with and without asthma. Results: Of the 460 children included in this study, those with asthma had higher vaccination rates (46.5%, 67/144) than those without asthma (30.4%, 96/316). Influenza A infections were diagnosed in 28 of 163 vaccinated children (17.2%) compared to 164 of 297 unvaccinated children (55.2%, p<0.001). Comparison of positive influenza diagnosis rates between vaccinated and unvaccinated children with and without asthma showed that unvaccinated children with asthma had an elevated odds ratio (13.235; 95% confidence interval [CI], 5.564-32.134) and that treatment for asthma exacerbations was needed in a larger proportion of unvaccinated children. Vaccine effectiveness against PDIA was 87% (95% CI, 78-93%) overall, 92% (95% CI, 81-96%) in children with asthma and 81% (95% CI, 63-91%) in children without asthma, respectively. Conclusions: The administration of an inactivated, split-virus, non-adjuvanted monovalent A(H1N1)pdm09 vaccine during the pandemic period reduced the number of physician-diagnosed influenza A infections and asthma exacerbations in children with asthma. Successful treatment of ileocolic intussusception with air enema reduction in an adult patientMatsui S; Kanemura T; Yokouchi Y; Kamiichi H; Kiriu N; Koike Y., Corresponding, Intussusception is a rare condition in adults, representing only 1% of all bowel obstructions. In adult cases, operative explorations are recommended to treat the bowel obstruction and to diagnose underlying diseases. The objective of the current case report was to describe the successful treatment of ileocolic intussusception with air enema reduction in an adult patient. A previously healthy 21-year-old woman had a 20-hour history of colicky abdominal pain and vomiting and was diagnosed as having idiopathic ileocolic intussusception by abdominal computed tomography. We treated the patient with air enema reduction under fluoroscopic guidance instead of an operative procedure. She received oxygen and intravenous midazolam to provide some degree of pain relief. Air was carefully pumped manually into the rectum, and the air pressure was monitored with a manometer. Because of air leakage from the rectum through the void to the outside the body, we continued to provide air to maintain the air pressure between 40 and 60 mm Hg. Three minutes after initiation of the air enema, when the patient experienced increasing abdominal pain and vomiting, the pressure was temporarily increased to greater than 100 mm Hg, and the air reached the terminal ileum. We considered the reduction successful and confirmed it with an abdominal ultrasound examination. We believe that air enema reduction is effective for treating idiopathic intussusception within 24 hours of symptom onset in young, previously healthy adult patients., W B SAUNDERS CO-ELSEVIER INC

An unusual association between growth hormone deficiency and a middle cranial fossa arachnoid cystKoike Y; Aoki N; Zhu Y., Lead, Middle cranial fossa arachnoid cysts (MFACs) are frequently asymptomatic. Here, we report the case of a 10-year-old boy with growth hormone deficiency (GHD) and an MFAC. His linear growth had followed the -2.5 SD line. On physical examination, his height was 120.8 cm (-2.6 SD), and the level of insulin-like growth factor 1 was low (87 ng/mL). GH provocative tests revealed GHD and brain magnetic resonance imaging (MRI) revealed an MFAC. We started treatment with GH replacement therapy (0.175 mg/kg/week). At the age of 12 years and 9 months, there were no interval changes in the features of the MFAC on the brain MRI. As his height was 145.5 cm (-1.7 SD) at the age of 13 years and 4 months, the therapy seems to be successful without sequelae. GH replacement therapy is suggested to be safe and effective to treat patients with GHD associated with arachnoid cysts., WALTER DE GRUYTER GMBH

As a result, the incidence of intraplacental chorlocarcinorna may be higher than previously estimated. Analysis of the susceptibility of CD57+ T cells to CD3-mediated apoptosisShinomiya N; Koike Y; Koyama H; Takayama E; Habu Y; Fukasawa M; Tanuma S; Seki S., After stimulation with anti-CD3 antibody in vitro, CD57(+) T cells showed a greater susceptibility to apoptosis than CD57(-)alphabetaT cell receptor (TCR)(+) T cells (regular alphabeta T cells). The apoptotic fraction of CD57(+) T cells showed an increased production of active caspase-3. An increase in both Fas expression and Fas-ligand (FasL) production was also observed in CD57(+) T cells, whereas the expression of survivin was suppressed in CD57(+) T cells compared to that of regular alphabeta T cells. CD57(+) T cells display a biased expansion of a few Vbeta T cell fractions in individuals, but such Vbeta T cells were not specifically susceptible to CD3-mediated apoptosis. The TCR expression level of CD57(+) T cells was much lower than that of regular T cells and anti-TCR antibody stimulation induced a smaller apoptotic proportion of CD57(+) T cells than did anti-CD3 antibody. Although the CD3epsilon expression levels were similar in both T cell subsets, the CD3zeta level of CD57(+) T cells was significantly higher than that of regular T cells. These results suggest that several apoptotic and anti-apoptotic molecules are involved in the CD3-induced apoptosis of CD57(+) T cells and raise the possibility that the imbalance in expression of the CD3epsilon and CD3zeta chains may also contribute to the susceptibility of CD57(+) T cells to undergo apoptosis., BLACKWELL PUBLISHING LTD

[Reviewed] Effect of 12 weeks of strenuous physical training on hematological changesKoike Y; Isozaki A; Nomura Y; Fujitsuka S., It is well known that sports and physical exercise affect the hemoglobin (Hb) concentrations; however, the factors affecting the hematological changes after intense exercise are still not fully elucidated. In the present study, we examined the hematological and biochemical parameters of nine young male soldiers participating in a 12-week ranger training program. The platelet counts before the training showed a strong inverse correlation with the Hb concentrations after the training (r = -0.829, p < 0.01). Furthermore, the Hb concentrations of the subjects whose platelet counts were more than 25.0 x 10(4)/mu L decreased significantly after the training (p < 0.05). Our findings suggest that an association exists between high platelet counts even within the normal range before strenuous physical training and the risk of a subsequent decrease in the Hb concentrations., ASSN MILITARY SURG US

千田礼子; 磯崎 淳; 古池雄治; 石井茂樹; 黒木康富; 藤塚 聡; 野々山恵章, Corresponding, 症例1:1歳9ヵ月男児.発熱と喘息を主訴とした.胸部X線で両側の肺門から下肺野にかけて気管支陰影の増強を認め,ディレクティジュンEZ RSウイルス(RSV)を用いた鼻汁中RSV抗原の検出は陽性であった.β刺激薬の吸入とアミノフィリンの持続静脈内投与,および酸素投与を行い,軽度症状改善後は徐放型キサンチン製剤の内服に変更した.RSV抗体価は入院前4倍であったが退院後は32倍に上昇し,RSV感染症と診断した.症例2:2歳4ヵ月女児.発熱と咳嗽を主訴とした.1歳7ヵ月時にRSV感染による気管支肺炎の既往があった.胸部X線で肺門から肺野にかけて気管支陰影の増強を認め,ディレクティジュンEZ RSVを用いた鼻汁中RSV抗原の検出は陽性であった.また,低酸素血症も認めた.酸素テントに収容し,輸液を行い,喘鳴は認めなかったがβ刺激薬の吸入を併用した.徐々に症状は改善し退院した.RSV抗体価は入院前64倍であったが退院後は256倍に上昇し,RSV感染症と診断した.症例3:2ヵ月男児.咳嗽を主訴とした.RSV抗原陽性で,胸部X線では軽度の気腫性変化を認めた.また,低酸素血症も認めた.酸素テントに収容し,輸液を行い,症状改善したため退院した, 防衛医科大学校雑誌編集委員会

Primary cervical spinal epidural extra-osseus Ewing`s sarcomaKogawa M; Asazuma T; Iso K; Koike Y; Domoto H; Aida S; Fujikawa K., Cases of primary spinal epidural Extra-osseous Ewing's sarcoma (EES) are rarely seen and a good prognosis for EES cannot be expected since a high incidence of local recurrence and metastasis frequently occur.
We present a case of cervical spinal epidural EES in a 7-year-old girl with long survival after tumour resection. She also received adjuvant treatment with peripheral blood stem cell transplantation (PBSCT). This management successfully improved her condition and she was in complete remission without neurological deficit 60 months after surgery. Such a good prognosis of EES in the cervical spine is very rare.
It is likely that magnetic resonance imaging (MRI) lead to a diagnosis of this rare type of EES at an early stage of the disease. PBSCT could be useful as an adjuvant to prolong the period of complete remission., SPRINGER WIEN

[Reviewed] Functional Vb repertoire characterization of human CD8+ T-cell subsets with natural killer cell markers, CD56+ CD57- T cells, CD56+ CD57+ Tcells and CD56- CD57+ T cellsTakayama E; Koike Y; Ohkawa T; Majima T; Fukasawa M; Shinomiya N; Yamaguchi T; Konishi M; Hiraide H; Tadakuma T; Seki S., We investigated the individual CD8(+) populations with natural killer (NK) cell markers (NK-type T cell); CD56 single positive (CD56)-T cells, CD56/CD57 double positive (DP)-T cells and CD57 single positive (CD57)-T cells in the peripheral blood. All NK-type T-cell populations expressed CD122 and intermediate levels of T-cell receptor (TCR; regular CD8(+) T cells are CD122(-) and express high levels of TCR). The number of both DP-T cells and CD57-T cells, but not CD56-T cells, gradually increased with age. All NK-type T-cell populations produced larger amounts of interferon-gamma than did regular CD8(+) T cells after stimulation with interleukin (IL)-2, IL-12 and IL-15. However, CD56-T cells and CD57-T cells but not DP-T cells showed a potent antitumour cytotoxity to NK-sensitive K562 cells, whereas only CD56-T cells showed a potent cytotoxity to NK-resistant Raji cells. Furthermore, although NK-type T cells produced large amounts of soluble Fas-ligands, their cytotoxic activities appeared to be mediated by the perforin/granzyme pathway. The oligoclonal or pauciclonal expansions of certain VbetaT cells were found in each NK-type T-cell population. The non-variant CDR3 region(s) for the TCRbeta chain(s) showed CD57-T cells and CD56-T cells to be derived from distinct origins, while the DP-T cell population consisted of a mixture of the clones seen in both CD56-T cells and CD57-T cells. [Reviewed] Characterization of bronchoalveolar lavage T cell subsets in sarcoidosis on the basis of CD57, CD4 and CD8Kurumagawa T; Seki S; Kobayashi H; Koike Y; Kanoh S; Hiraide H; Motoyoshi K., T cells expressing CD57 (a natural killer cell marker) with interferon-gamma (IFN-gamma) producing capacity increase under various conditions. CD57(+) T cells are also present in the bronchoalveolar lavage fluid (BALF) of sarcoidosis, and several phenotypical and functional analyses of these cells have been reported. In the present study, BALF T cells obtained from 52 patients with sarcoidosis were classified further into CD4(+)CD57(+) T cells, CD4(+)CD57(-) T cells, CD8(+)CD57(+) T cells and CD8(+)CD57(-) T cells and their phenotypes and functional characteristics were assessed. Substantial proportions of these T cell subsets expressed natural killer cell markers CD161 and CD122. The biased expansion of Vbeta2 T cells was observed in both CD4(+)CD57(+) T cells and CD4(+)CD57(-)T cells in BALF from most patients, while the expansion of other Vbeta T cells was also observed in some patients. Unexpectedly, the biased expansion of certain Vbeta T cells was also seen in either CD8(+)CD57(-) T cells or CD8(+)CD57(-) T cells, while the expanded Vbeta T cells in CD8(+) T cells differed substantially among individuals. BALF T cells showed a remarkably lower T cell receptor (TCR) intensity than that of peripheral blood T cells. Both CD8(+) T cell subsets in BALF of sarcoidosis expressed the intracellular perforin/granzyme B, while all four subsets expressed intracellular IFN-gamma after in vitro activation, and CD4(+) T cells, especially CD4(+)CD57(-) T cells, expressed tumour necrosis factor-alpha. These findings indicate that CD57(+) T cells as well as CD57(-) T cells in the BALF are phenotypically and functionally different from peripheral blood T cells and may play an important role in the Th1 dominant state and inflammation in pulmonary sarcoidosis., BLACKWELL PUBLISHING LTD

Activation of human T cells with NK cell markers by staphylococcal enterotoxin A via IL-12 but not IL-18Ami K; Ohkawa T; Koike Y; Sato K; Habu Y; Iwai T; Seki S; Hiraide H., We have reported recently that mouse liver NK cells and NK1.1(+) T cells were activated by bacterial superantigens via the IL-12 production from Kupffer cells. In the present study, we examined the effect of staphyloccoccal enterotoxin A (SEA) on human T cells with NK cell markers, CD56 or CD57 (NK-type T cells). After stimulating peripheral blood mononuclear cells (PBMC) with SEA, PBMC produced a large amount of IFN-gamma and acquired a potent antitumour cytotoxicity. The in vitro depletion of either CD56(+) TCR- NK cells, CD56(+) T cells or 57(+) T cells from PBMC significantly inhibited the IFN-gamma production from PBMC. When purified NK-type T cells, NK cells and regular T cells were cultured with monocytes and SEA they all produced IFN-gamma, while the IFN-gamma amounts produced by both NK-type T cells were greater than those produced by NK cells. NK cells as well as CD56(+) T cells showed cytotoxicity against NK-sensitive K562 cells, whereas both NK-type T cells showed a more potent cytotoxicity against NK-resistant Raji cells than did NK cells. The IFN-gamma production from each population as well as from whole PBMC was greatly inhibited by anti-IL-12 antibody but not by anti-IL-18 antibody. The antitumour cytotoxicity of whole PBMC was also significantly inhibited by anti-IL-12 antibody while the SEA-induced proliferation of PBMC was not affected by anti-IL-12 antibody. Furthermore, SEA-activated NK-type T cells as well as NK cells showed cytotoxicities against vascular endothelial cells. In this study, we showed that CD57(+) T cells in responder cells were essential for effective IFN-gamma production in allogeneic MLR due partly to the augmentation of the alloresponse of regular T cells. Furthermore, IFN-gamma production in MLR correlated with the proportions of CD57(+) T cells in PB regardless of the responders' age. We also showed that the extent of the expansion of CD57(+) T cells in paediatric patients after haematopoietic stem cell transplantation (HSCT) was markedly lower than that in adult patients. In addition, CD57(+) T cells purified and activated with a combination of cytokines showed a greater cytotoxicity than regular T cells against human umbilical vein endothelial cells. Because IFN-gamma production in one-way MLR is a useful predictor of graft-versus- host disease (GVHD), especially in the acute phase that occurs after allogeneic HSCT, our findings suggested that CD57(+) T cells play a role in the development of GVHD and thus may explain the reason as to why a higher donor age is associated with an increased risk of developing GVHD while, in addition, the incidence of severe GVHD in paediatric patients is lower than that in adult patients., BLACKWELL PUBLISHING LTD

[Reviewed] Age-associated augmentation of the synthetic ligand-mediated function of mouse NK1.1+ T cells: their cytokine production and hepatotoxicity in vivo and in vitroInui T; Nakagawa R; Ohkura S; Habu Y; Koike Y; Motoki K; Kuranaga N; Fukasawa M; Shinomiya N; Seki S., We recently reported that the direct antitumor effectors in the liver induced by alpha-galactosylceramide (alpha-GalCer) are NK cells that are activated by the IIFN-gamma produced from NK1.1 Ag+ T cells (NKT cells) specifically stimulated with alpha-GalCer, whereas NKT cells cause hepatocyte injury through the Fas-Fas ligand pathway. In the present study, we investigated how mouse age affects the alpha-GalCer-induced effect using young (6-wk-old), middle-aged (30-wk-old), and old (75-wk-old) mice. The serum IFN-gamma and IL-4 concentrations as well as alanine aminotransferase levels after the alpha-GalCer injection increased in an age-dependent manner. An alpha-GalCer injection also induced an age-dependent increase in the Fas ligand expression on liver NKT cells. Under the stimulus of alpha-GalCer in vitro, the liver mononuclear cells from old and middle-aged mice showed vigorous proliferation, remarkable antitumor cytotoxicity, and enhanced production of both IFN-gamma and IL-4 in comparison to those of young mice, all of which were mediated mainly by NK1.1(+) cells. Furthermore, liver mononuclear cells from old mice stimulated with alpha-GalCer showed a more potent Fas-Fas ligand-mediated cytotoxicity against primary cultured hepatocytes than did those from young mice. Most a-GalCer-injected old mice, but no young mice, died, while anti-IFN-gamma Ab. pretreatment completely inhibited mouse mortality. However, alpha-GalCer-induced hepatic injury did not improve at all by anti-IFN-gamma Ab treatment, and the Fas-ligand expression of liver NKT cells did not change. [Reviewed] Primary amenorrhea in an 18-year old patient with premenarcheal onset anorexia nervosa after reaching an appropriate body weightKoike Y; Fujitsuka S; Sato K; Motegi A; Ohkawa T; Sekine I., Inappropriate body weight loss during early adolescence especially due to anorexia nervosa (AN) and even fear of obesity is known to cause severe endocrinological disturbances, such as delayed puberty and growth retardation. Nevertheless, after achieving adequate weight and then recovering the GH-IGF-1 axis, how long menarche is delayed has yet to be elucidated. We herein describe an 18-year old patient with premenarchal onset AN. It took 4 years for her to achieve menarche after attaining an appropriate body weight and recovering her linear growth. We suggest that the hypothalamopituitary-gonadal axis rather than the GH-IGF-I axis could thus be severely damaged by an inappropriate weight loss for several years during early adolescence in association with AN pathophysiology. The mechanism of a defective IFN-gresponse to bacterial toxins in an atopic dermatitis model, NC/Nga mice, and the therapeutic effect of IFN-g, IL-12, or IL-18 on dermatitisHabu Y; Seki S; Takayama E; Ohkawa T; Koike Y; Ami K; Majima T; Hiraide H., NC/Nga (NC) mice raised under conventional conditions (Conv. NC mice) spontaneously develop dermatitis similar to human atopic dermatitis, whereas NC mice raised under the specific pathogen-free conditions do not develop dermatitis. In the present study, we show that the representative Th1 cytokine, IFN-gamma levels in the sera of NC mice, injected with either staphylococcal enterotoxin B or endotoxin (LPS), to be severalfold lower than those of normal mice. The low IFN-gamma response to staphylococcal enterotoxin B was correlated to the lack of regular V beta8(+) T cells and V beta8(+) NK T cells, and the low IFN-gamma response to LPS was correlated to an impaired IL-18 production of macrophages. The CD3-stimulated IL-4 production from liver and spleen T cells from Conv. NC mice in vitro was greatly augmented. The serum IL-4 levels of untreated Conv. NC mice also were higher than those of normal mice and specific pathogen-free NC mice. Treatment of Conv. NC mice either with IFN-gamma, IL-12, or IL-18 twice a week from 4 wk of age substantially inhibited the elevation of the serum IgE levels, serum IL-4 levels, and dermatitis, and IL-12 or IL-18 treatment also reduced the in vitro IL-4 production from CD3-stimulated liver T cells. The systemic deficiency in the Th1 response to bacterial stimulation thus leads to a Th2-dominant state and may induce an abnormal cellular immune response in the skin accompanied with an overproduction of IgE and a susceptibility to dermatitis in NC mice., AMER ASSOC IMMUNOLOGISTS

[Reviewed] Systematic characterization of human CD8+ T cells with natural killer cell markers in comparison with natural killer cells and normal CD8+ T cellsOhkawa T; Seki S; Dobashi H; Koike Y; Habu Y; Ami K; Hiraide H; Sekine I., We investigated the function of CD56(+) CD8(+) T cells (CD56(+) T cells) and CD56(-) CD57(+) CD8(+) T cells (CD57(+) T cells; natural killer (NK)-type T cells) and compared them with those of normal CD56(-) CD57(-) CD8(+) T cells (CD8(+) T cells) and CD56(+) NK cells from healthy volunteers. After the stimulation with immobilized anti-CD3 antibodies, both NK-type T cells produced much larger amounts of interferon-gamma (IFN-gamma) than CD8(+) T cells. Both NK-type T cells also acquired a more potent cytotoxicity against NK-sensitive K562 cells than CD8(+) T cells while only CD56(+) T cells showed a potent cytotoxicity against NK-resistant Raji cells. After the stimulation with a combination of interleukin (IL)-2, IL-12 and IL-15, the IFN-gamma amounts produced were NK cells greater than or equal to CD56(+) T cells greater than or equal to CD57(+) T cells > CD8(+) T cells. The cytotoxicities against K562 cells were NK cells > CD56(+) T cells greater than or equal to CD57(+) T cells > CD8(+) T cells while cytotoxicities against Raji cells were CD56(+) T cells >CD57(+) T cells greater than or equal to CD8(+) T cells greater than or equal to NK cells. However, the CD3-stimulated proliferation of both NK-type T cells was smaller than that of CD8(+) T cells partly because NK-type T cells were susceptible to apoptosis. In addition to NK cells, NK-type T cells but not CD8(+) T cells stimulated with cytokines, expressed cytoplasmic perforin and granzyme B. Furthermore, CD3-stimulated IFN-gamma production from peripheral blood mononuclear cells (PBMC) correlated with the proportions of CD57(+) T cells in PBMC from donors. The aim of this study was the further characterization of these cells in the liver with or without hepatitis C virus (HCV) infection. Liver mononuclear cells (MNC) were isolated from liver specimens obtained from the patients during abdominal surgery. In addition to a flow cytometric analysis, liver MNC and PBMC were cultured with the immobilized anti-CD3 Ab, IL-2, or a combination of IL-2 and IL-12 and their IFN-gamma production and the antitumor cytotoxicity were assessed. The liver MNC of HCV (-) patients contained 20% CD56(+)T cells whereas the same proportions decreased to 11% in chronic hepatitis livers and to 5% in cirrhotic livers. The proportion of NK cells also decreased in the cirrhotic livers. On the other hand, the populations of these cells in PBMC did not significantly differ among patient groups. The IFN-gamma production and the cytotoxicity against K562 cells, Raji cells, and a hepatocellular carcinoma, HuH-7 cells, greatly decreased in the cirrhotic liver MNC, In contrast, the cytotoxicity in PBMC did not significantly differ among the patient groups and was lower than that in the liver MNC of HCV (-) patients. CD56+T cells and NK cells but not regular T cells purified from liver MNC cultured with cytokines showed potent cytotoxicities against HuH-7 cells. These results suggest that a decreased number of CD56(+)T cells and NK cells in cirrhotic livers may be related to their susceptibility to hepatocellular carcinoma., W B SAUNDERS CO

Circulating soluble selectins in Kawasaki diseaseTakeshita S; Dobashi H; Nakatani K; Koike Y; Tsujimoto H; Hirayama K; Kawamura Y; Mori K; Sekine I; Yoshioka S., To investigate the significance of circulating adhesion molecules associated with leucocyte-endothelial cell interaction in Kawasaki disease (KD), serum levels of soluble E-, P-, L-selectin (sE-, sP-, sL-selectin), and vascular cell adhesion molecule-1 (VCAM-1) were measured in 16 patients with KD, eight with other febrile diseases, six with Henoch-Schonlein purpura (HSP), and 10 healthy children using an ELISA. Serum sE-selectin levels from patients in the acute phase of KD were significantly higher than from those in other groups (P < 0.01). The levels of sP-selectin in the subacute phase of KD were significantly higher than in other groups (P < 0.01). Serum sL-selectin levels tended to rise in the convalescent phase of KD. There were also significant correlations between sE-selectin levels and C-reactive protein (r = 0.80, P < 0.001), and between sP-selectin levels and platelet counts (r = 0.57, P < 0.0001) in KD patients. These data indicate that circulating soluble forms of three selectins may have different kinetics during the clinical course of KD, suggesting that they may reflect its inflammatory process., BLACKWELL SCIENCE LTD
